The game is much less than it was 5 years ago, but it's still reasonably active — there are literally millions of player-made levels in total (over 37 million apparently) and even now there are always plenty of levels in the 'new' category, meaning new levels are still being made daily.
Personally I've played the game for hundreds of hours and although I play less now and am waiting for smm3, I still play a bit every week or two.
In terms of making levels, it is very much up to you — the game was designed to have levels created either way. It is generally considered to be a bit easier to make levels with the touch controls (especially with a stylus) but there are plenty of makers who prefer using the controllers with docked even though it is a smidge more cumbersome.
You can use your fingers, but a stylus certainly helps. Nintendo has occasionally included styluses with games (including smm2), but I don't see them on the website at the moment. Generally, any basic stylus meant for tablets should work. I know Amazon has some fairly cheap ones which specify they work with Switch as well as some even cheaper generic ones which likely also work for with Switch. Personally I had a stylus included with my Switch travel case and it works well but it is clearly just a generic stylus mean for iPads or whatever.

Yea, it's much less than at launch obviously, but there's still a community around it and new levels are being made all the time. Some of my favorite Youtubers to watch are Maker 2 people, and they're still going strong.
