What we know:
1) It's been scheduled for a 90 minute presentation/Q&A session.
2) It's directed at Western gamers.
Given that, in order of likeliness, it's pretty much guaranteed to be:
1) Traditional system-seller in the West.
2) New IP from veteran developer/studio.
3) New partnership with 3rd party, with their first output announced for 3DS.
A traditional system seller won't need that much explaining. Same with most new IP's. Don't think they would give a 3rd party that level of exposure.

I was hoping for an AR Warioware. It could even be named wARiowARe! also considering that the 2DS came out, i wouldn't think that 3D would be the focus.

Alright, factually looking at this, it is a new game, previously unannounced, which will have 1.5 hours of gameplay footage. It is important enough not to recieve a direct, but A WHOLE THING AT E3. So I doubt it's anything anyone has ever seen before. Ever. If it is, then it wouldn't require this long to explain, at all.
Whatever it is, expect it to be, well, in depth. There's no way they'll show something simple for that long. So a new complex-ish game on the 3DS, previously unannounced and important enough to recieve a major e3 timeslot.
The only things I can imagine woud be:
Even though its its own platform, their QOL thing could somehow tie in with the 3DS.
A new preexisting title taken in a completely new direction, which isn't a new IP, but would require time to explain. Some ideas would be, yes, Metroid (well after Other M it needs to impress again), FZero, Star Fox or really anything that is not majorly important or well recently popular so it can be revamped.
This is what I believe we will see... a new old ip, redone ala Kid Icarus. Think about it. There are tons of amazing forgotten games which have been overlooked. What do I expect? A new Baten Kaitos. Think about it... it was a Gamecube game which was a good game, then never touched again. But now, Bandai Namco and Nintendo are clearly best friends, working on smash bros. But that's not all, MONOLITH SOFT worked on these games, and they happen to have a new 3DS game kined up, unannounced. hint hint This would almost make too much sense... introduce a good series to a ton of people who now own a 3DS, and revamp it to appeal to the masses. Then, boom, there's a great series to add to the library. They could spend the time discussing the old game, then the new, then comparing the two to show improvements. If they did this, I'd applaud Nintendo.

Anyway, this is a Q/A presentation. It and the Smash Bros one will likely be like the Pokemon X/Y presentation last year. It starts with a presentation about the game where they go into details, followed by a Q&A session afterwards. So no, there won't be 90 minutes of gameplay.

If it is a new Rhythm Heaven (which I doubt), I really hope they don't use touch controls like the DS game. Imprecise touch screens and motion controls don't belong in a a game like that.
The first Rhythm Heaven game wasn't even out ten years ago...
